• Active Duty Military Fee Waiver Request 
• Enrollment Verification Request (Login to PAWS, click on STUDENT PORTAL, then ENROLLMENT VERIFICATIONS under ADDITIONAL LINKS Menu) 
• FERPA Waiver (Family Educational Rights and Privacy Act)/Student Release of education records)
• Graduation Application (Make an appointment with your Academic Advisor before completing!)
• Major Change Form
• Add/Change a Minor or Certificate 
• Name Change Form (Address or phone number changes can be done through PAWS under Personal Information)
• Overload Request Form (request an increase in hours for the semester)
• Participation in Commencement (Walk No Diploma) Form 
• Readmission Application (Use this application if you have been out of school for more than one year or returning after suspension)
• Reinstatement for Non-Attendance Form

• Transient/Letter of Good Standing 
• Veteran Affairs Enrollment Certification 
• Withdrawal from a Course  (Use to request to drop/withdraw from individual courses)
• Withdrawal from the Semester (Use to withdraw from all classes in a semester)
• Withdrawal due to Hardship  (Use this form only if you are requesting a withdrawal after the withdrawal deadline due to non-academic, extenuating circumstances)
